Case-based Review of Response to Neoadjuvant Chemotherapy and Why Molecular Subtypes Are Important

The session will start with a summary of the breast cancer molecular subtypes and their imaging features in multi-modality breast imaging. Adjuvant vs. neoadjuvant systemic therapy, including hormonal therapy, will be defined. The types of imaging responses will be defined and illustrated, and suggestions on report documentation will be provided. A case-based review will highlight the typical imaging patterns of treatment response, which can be seen with TNBC, HER2+, and luminal tumors. Important imaging findings, including distance from the nipple, involvement of the skin and chest wall, and management of the axilla, will be illustrated. The session will conclude with a discussion of the challenges of localizing breast masses and lymph nodes that are no longer visualized after neoadjuvant systemic therapy.
